Integrative Care Provider

Old Town, Fort Collins, CO.

  • This position is for a full-time position for a qualified caregiver, with an initial period of employment of 1-3 years.

  • Total starting annual compensation for an individual with prior experience would be ~$72,300, which would include ~$45,000 in direct pay + ~$21,600 (rent, shared groceries, and all utilities) + ~$5400 in other benefits (paid time off, health insurance, retirement match, relocation expenses, bonus for working holidays, and fees for training expenses, such as first aid, CPR, or other continuing education).

  • This is an invitation-only position and requires prior communication. Any proposed contract would be individualized based on the number of hours worked per week, starting hourly rate, and approximately 50 weeks per year).

OVERVIEW/Working schedule:

  • Weekly, this position includes ≥5 night shifts/week plus 30 other work hours.

    • The night shift requires only around five minutes of hands-on assistance. The rest of the night, you would be free to sleep.

    • Most other work would be performed at defined times (e.g., 6-9 am; 11 am-12 pm, 5:30-7 pm, and 9-10 pm), providing free time during the day.

  • Ongoing guidance will be provided by James and his designee(s) from the care team.

The exact schedule, job duties, and procedures will change occasionally. A typical week might include several morning and meal shifts at defined times, as well as a diverse variety of other tasks (see “Tasks” below), on a flexible schedule, including therapeutic recreation and exercise, assistance with shopping, errands, transport to/from work or appointments, and other outings. Regular opportunities for pay raises will be provided (see details below).

JOB LOCATION:

A mixed-use building in Old Town, Fort Collins, Colorado, just one block from Washington Park and Lee Martinez Parks, and a mile north of Colorado State University on the Mason corridor, which has bicycle and pedestrian pathways, and just across the street from the Downtown Transit Center and public transportation—the Max bus rapid transit and other options going to Boulder and Denver. Fort Collins, Colorado was recently ranked the best place to live in the USA. It is a beautiful, modern, spacious, 95% furnished 1,056 square foot, 2 bed, 2 bath unit with secure entrances, exits, elevator, and underground parking garage (with storage and bicycle parking). The unit has high ceilings and large closets. A large kitchen, dining, living area separates master bedroom from second bedroom and bathroom, providing privacy. There’s a nice view – above the trees and most buildings – and large windows and a balcony, providing nice views of old town and the mountains! There’s a state-of-the-art kitchen with stainless steel appliances–gas stove/range, microwave, toaster oven, dishwasher, washer and dryer in unit, and central heat and air conditioning, ceiling fans, high-speed Internet, and Wi-Fi.


tasks:

The provider will perform a diverse variety of tasks in the home environment, including to evaluate client needs to create a custom treatment plan; use a variety of therapeutic methods and hands-on support with activities of daily living (diet, physical activity, and socioenvironmental factors) to improve quality of life; teach client and caregivers coping mechanisms related to relationships, physical, mental, and emotional health/life issues, and provide resources to overcome problems and reach personal goals; monitor client progress and adjust treatment plan as needed; and coordinate with other healthcare providers, agencies and community resources to create a thorough treatment plan. Tasks include:

  • Safely transfer between wheelchair/bed using patient lift (not physical lifting).

  • Perform regular bathing, dressing, hygiene, grooming, and toileting (bowel program).

  • Perform routine care of urinary drainage devices and apply and irrigate catheter.

  • Thoroughly/accurately observe, document, report changes in health condition.

  • Maintain inventory sheet of medical supplies and medications.

  • Assist with positioning, exercises, massage, and range of motion/stretching.

  • Assist with meal planning, shopping, preparation, assist with eating, storage, and cleanup.

  • General housekeeping: cleaning, laundry, and ironing, etc.

  • Transport/accompany (for appointments and outings) and assist as necessary.

  • Other tasks in the scope of the work agreement acknowledged by both parties.

Needed guidance, training, and access will be provided by James Peth and his designee(s). The live-in caregiver will receive ongoing assistance from other caregivers.


ABOUT YOU / QUALIFICATIONS: 

You are seeking a full-time position and a place to live and a roommate/friend. I need someone responsible who I can rely on. We’d be sharing a home, so we should enjoy each other’s company and have overlapping interests. You should be kind, intelligent, caring, empathetic, compassionate, respectful, and willing to learn/help, calm under pressure and in case of emergency, flexible, mature, emotionally stable, self-sufficient, dependable, clean/organized, and goal/detail oriented, and have a good/positive attitude, good observation and communication skills, and physical endurance.

  • At least 18 years old.

  • Authorized to work in the United States.

  • Bachelor’s degree and licensure in relevant field + some professional training and experience.

  • Available to work night shift (most weekdays, weekends, and holidays); absences infrequent/planned to ensure coverage.

  • Have current driver’s license and safe driving record.

  • Speak and write in English at an intermediate or advanced level.

  • Must be able to pass a complete drug screen and a background check per local law/regulations.

  • Some experience in a relevant health/medical field and/or patient care.

A provider with relevant experience with positive psychology, adult learning theory, motivational interviewing, mindfulness practices and related mind-body relaxation therapies may assist the client with physical, mental, and emotional health/life issues and also serve as a consultant/counselor to assist with health and life coaching clients to help with self-identified health and well-being goals. This will incorporate evidence-based coaching and positive psychology, adult learning theory, motivational interviewing, and new findings in neuroscience to elicit behavior change, enhanced decision making, and better use of resources. The emphasis is on well-being, strengths-based thinking, and cultivating the approaches to happiness. Mindfulness practices and related mind-body relaxation therapies such as guided imagery, and conscious breath work is used in an effort to reduce stress, depression, anxiety or pain. Motivational Interviewing is also used to empower clients to discover their own reasons for change, discern discrepancies about actions and intentions, and resolve ambivalence at their own self-determined pace. This is accomplished through empathic listening and supporting the growth of self-efficacy and nurturing of nascent intrinsic motivation.


Pay: 

Wages will be re-assessed after 80 hours of work (after completion of necessary training). Raises will be based on demonstrated performance and adherence to the following standards and/or relevant continuing education, and at James Peth’s discretion: Is prompt/timely to all shifts; Demonstrates ability to perform all responsibilities using provided resources and with an attitude of cooperation and professionalism; Maintains harmonious relationship and communication with James and others, providing support as requested to ensure quality care; Maintains confidentiality of ALL information (health-related or otherwise). Communicates/reports shift notes as requested and reports any important details; Regularly checks in to insure comfort, dignity, and privacy; Identifies when help is required in providing care and seeks it appropriately, recognizing own limitations; Communicates clearly and tactfully with James and his designees; Demonstrates knowledge of and adheres to all food safety procedures; Organizes work to complete job tasks within the scheduled shift and responds promptly to change in workload, adjusting assignments to achieve optimal productivity and efficiency; Maintains a clean, orderly environment.
